CHATFIELD, Sally Ann (nee Carey). Passed away peacefully on 4 August 2015 at Mercy Hospice, surrounded by her family. Loved wife of Greg for 34 years, adored mum to Amy and Kate. The family would like to give heartfelt thanks to everyone who supported her on her journey. A special thanks to Oncology 161, Dove House and Mercy Hospice. A service to celebrate Sally's life will be held on Saturday 8 August at St Andrews Anglican Church, 116 Selwyn Avenue, Mission Bay at 1.30pm.
